Either the coolant temp sensor is bad, disconnected, or has a broken wire. If the PCM can not read the engine temp then it turns the fans on as a fail-safe condition. Start with the checking the temp sensor and we'll go from there.

EDIT: BTW.....Is the SES light on? You should probably be seeing a SES OBDII code if the temp sensor is not working properly.

Its your coolant temp sensor and maybe thermostat.

I had this issue. Since you have to remove things to get to the coolant temp sensor. I just did the thermostat at same time. Once all back together car was fine.

Yeah, I was gonna say... having no coolant temp reading and fans kicking on right away should be a dead giveaway. Check your coolant temp sensor and the wires going to it.

Also had these symptoms and I was low on coolant, so you might want to check that as well, just to be safe.
